Short Term: Maternity Grant
MATERNITY GRANT (One Lump Sum)
Maternity Grant is a lump sum payment of $300.00 made upon confinement. Maternity Grant may be paid to an Insured Man if his wife does not qualify for the benefit.
Maternity Grant of $300.00 is payable per child in one calendar year.
HOW TO QUALIFY
YOU MUST |

1. Be an insured woman |
2. Have been confined |
3. Have 50 paid contributions |
4. Have 25 paid contributions in the 50 weeks before confinement |
5. Or | Be an insured man |
6. Have 50 paid contributions |
7. Have 25 paid contributions in the 50 weeks before spouse’s confinement |
8. WIFE, (married or common-law) has been confined |
9. WIFE does not qualify for the benefit |
ADDITIONAL REQUIREMENTS |

1. The insured woman must have claimed Maternity Benefit on the prescribed form (MB1) |
2. A male claimant (insured man) must make claim on the prescribed form (MB5) |
3. MB5 must be fully completed and signed by the insured contributor |
4. Must present Birth Certificate or certificate of registration of birth of child, showing claimant as the father |
5. Employer must declare claimant’s employment status. Record the salary or wages (SM2/MB form)of the insured man 50 weeks before confinement |
6. Submit claim within 3 months after date of confinement |
7. If claim is not submitted within 3 months after date of birth, a good reason must be given by the insured person and recorded on the MB5 |
