HOW TO QUALIFY

For death of an insured contributor’s dependent (spouse or child), the insured contributor must have no less than 150 contributions | Proof of payment of funeral cost by way of original receipts in the name of the applicant or proof of liability to pay by way of original bills or pro forma invoice in the name of the applicant. | |
The dependent child of the deceased insured contributor must be unmarried and under the age of 16 years, or 21 years if receiving full-time education at the time of death | Claim should be made within 6 months from the date of death of the person | |
The spouse can be either by marriage or proven common-law | If claim is made more than one year after death, no sum shall be paid |
