Dear valued pensioner,
To ensure that you continue to recieve your pension monthly, please be sure to fill out a life declaration form and hand it in to the Social Security Board every six months. We ask that you fill it out during June and December yearly, and the form will be valid for the proceeding six months.
Please fill out form A if you are receiving a Retirement Benefit Pension and over the age of 65 years OR receiving a Disability Benefit Pension regardless of age.
Please fill out form B if you are under the age of 65 years and are receiving a Retirement Benefit Pension OR an Invalidity Pension. Kindly ensure that you declare your employment status.

Please fill out form C if you are receiving a Survivor's Benefit Pension or Death Benefit Pension. If you have children who are 16 years and not yet 21 years of age, proof of education must be submitted.
- Submit proof of education for children who are attending high school in September of each year.
- For children attending sixth form or university, proof of education must be submitted in January and September of each year.

If you are a parent receiving Survivor's Benefit or Death Benefit Pension, kindly fill out form D.
If you are receiving Non-Contributory Pension (NCP), kindly fill out form E. This form must be filled out by the pensioner and not the person who is authorized by the pensioner to collect.
